Vista Academy Middle School Applications Guide lines 1. Students must complete an online application or print out the application and return to Vista Academy at 4800 Telluride, Student Union, Building 1; Denver, CO 80249. 2. Students must complete a three-five minute video clip demonstrating who the student is for the admissions committee. The video clip can be submitted attached to the completed online application or submitted on a thumb drive or disc with a completed hard copy of the application. 3. Students will have to submit three recommendations; two must be from a teacher and/or school staff. The third recommendation must be from an adult not related to the applicant outside of school, which can include mentors, social worker, mental health therapist, coach, extra-curricular activity sponsor and etc…). 4. All application materials must be submitted via online. If the student is submitting a hard copy of the application it must be turned in with all of the application materials to the Vista Academy Office by 4pm on the day that the application process closes.
